TERMS AND CONDITIONS GOVERNING ATM-SAVINGS ACCOUNT

The ATM Account. Only an individual (Single Account) can open an ATM Account.

ATM Card Ownership. The depositor acknowledges that the Bank’s ATM card is and shall remain the property of the Bank and is subject to revocation and/or repossession by the Bank upon notice for violation of any terms and conditions stated herein and for any other reason (s) which in the opinion of the Bank would prejudice its interest or that of the client arising from the continued use of the said card. The depositor further agrees that the Bank may program the ATM to retain the card automatically at any time without prior notice to the depositor.

Personal Identification Number (PIN) The ATM card is for the exclusive use of the cardholder and is not transferable. The PIN designated for the exclusive use of the Cardholder shall not be disclosed to any other person in all circumstances. Any transaction entailing the use of the PIN shall in all circumstances and at all times, with or without the Cardholder’s authorization, is conclusively binding on the Cardholder.

Deposits to ATM-Savings Account whether cash or check shall be made only through the bank’s teller. Check deposit will be made available for withdrawal upon clearance of the said check deposit subject to the Bank’s existing rules and regulations regarding check deposit.

Withdrawals. The Bank shall impose a limit to the amount that can be withdrawn from the ATM regardless of sufficiency of balance of the Cardholder’s account. For now withdrawals through ATM are subject to a maximum limit of P20,000.00 in accumulated total per day and a maximum limit of P10,000.00 per withdrawal transaction and in no case shall not exceed a total of ten (10) withdrawal transactions per day Maintaining balance is pegged at P100.00

Payments. The Cardholder hereby authorizes the Bank to debit his account without prior notice for the amount involving :
- The use of ATM Card for merchandise purchased from services rendered by Nation link accredited institutions, and
- The use of ATM Card for billings of accredited institutions.
In accepting bills / merchandise payment, the Bank hereby acts as the Conduit of the Cardholder and Payee institution/ merchant and shall therefore not be answerable for discrepancies committed by the Cardholder nor shall be Bank be held liable for any action which may arise between the Payee and the Cardholder, the latter hereby agreeing to bind him and undertake to indemnify and save the Bank free and harmless from any and all losses, claims, damages, liabilities, fees, expenses, etc.
- Fund Transfers. Any transfer of funds through ATM shall be deemed final and settled once the system has successfully executed the instruction/transaction.
- Responsibility of the Cardholder. The Cardholder shall accept full responsibility for all transactions made by the use of the Card with or without his knowledge or authority, and shall accept the Bank’s record of transactions as conclusive and binding for all purposes. The Cardholder further agrees to waive any and all remedies against the Bank in connection thereto.
- Lost/Stolen Cards. In the event of loss of theft of the ATM Card, the Cardholder shall immediately notify the Bank in writing through the ATM Maintenance Form and Affidavit of Loss. Prior to the receipt of such written notice by the Bank, all transactions generated by the use of the Card shall be conclusively binding on the Cardholder. A replacement card may be issued at a fee charged by the Bank. In case the Cardholder wants to lift the suspension of transactions of the ATM Card reported lost/ stolen, he/ she must inform the Bank in writing through the ATM Maintenance Form. After submission of such notice, the ATM Card shall not be tagged as lost/ stolen anymore.
- Retention of Cards. The bank shall cause the retention of the ATM Card by the machine for any of the following reasons:
- When the Bank has terminated the deposit account or this service agreement with the Cardholder.
- When the ATM Card has been reported lost/ stolen
- When the ATM Card is damaged.
If the reason for the retention of the Card is letter (b) above, the Bank may reissue the same ATM Card to the Cardholder if he so requests, subject to the conditions the Bank may impose, if any.

SCHEDULE OF FEES AND OTHER CHARGES
1. Inter-branch over the counter deposit
Up to P40,000.00 P50.00
Over P40,000.00, 1/8 of 1% of amount of deposit
2. Issuance of new card in lieu of lost/stolen P150.00
3. Maintenance/Convenience fee for using our ATM-terminal

I hereby agree to be governed by the terms and conditions of the COOPERATIVE BANK OF QUEZON PROVINCE as well as the rules and regulations of Bangko Sentral ng Pilipinas relative to the establishment and operation of my account.
